Your First Visit
Plan to spend about an hour with us during your first visit depending on what you come in for. We will take a careful medical history, physical examination and possibly perform some sort of testing (skin testing, spirometry, etc.) We will also begin the process of treating your condition with appropriate medications and providing education regarding avoidance of possible allergens.
Please remember to bring your insurance card, a picture I.D. and any necessary referrals. To ensure you get the best care possible, please also bring a list of your current medications and copies of any blood tests or X-rays that may have been done in relation to your reason for seeing us.
In order to be skin tested on your first visit, please refrain from taking any antihistamines (allergy pills) for at last 5-7 days prior to your appointment.

Tuesday 9AM – 5PM, closed from 12:30-1:30PM for lunch
Wednesday 9AM – 5PM
Thursday 9AM – 5PM
Friday 9AM-1PM

For your convenience, we have provided common patient forms for download. Feel free to complete these forms prior to coming to the office to save yourself time at the office! We also provide easy check-in with our user friendly patient kiosk in the office if you decide not to download forms at home.
